Band Forms

The following forms are required for each student (new and returning) and must be completed every year. If you have any questions about these forms, please contact centaurushs.band@gmail.com.

2025 - 2026 School Year Forms

• Band Form A - Student Information
• Band Form B - Authorization to Treat a Minor
• Band Form C - Parental Consent for Travel
• Band Form D - Instrument Rental Agreement. Only required if renting an instrument from Mr. V. Please complete one form per instrument if you are renting multiple instruments.
• Band Form E - Marching Band Uniform Responsibility
